Hospice Services

The goal of hospice care is to provide a comfortable, meaningful and peaceful end-of-life experience to those who are terminally ill. As such, the vast majority of people on hospice receive services at home, in nursing facilities, and assisted living facilities  – as opposed to special in-patient facilities.

Hospice services offer a means of improving the quality of life, like palliative services, but hospice care is for people who have decided not to pursue any further curative treatment. Services are reserved for those with a prognosis of six months or less, and care is provided by a team of professionals. The Suncrest hospice care team members include:

If your loved one meets the eligibility requirements, coverage for home health services from the Suncrest team may be available through Medicare, Medicaid or private insurance. To qualify, a person must:

  • Be under the care of a physician
  • Require skilled rehabilitative services
  • Have a doctor certify that they’re homebound

Palliative Services

Palliative care is designed to improve the quality of life for patients who are suffering from serious illnesses and those living with chronic diseases like cancer, COPD, congestive heart failure or Alzheimer’s disease. Your loved one doesn’t need to be at the end of life to receive palliative care, and they can still pursue curative treatments. Contrary to popular belief, this form of home health care is focused on living, not dying. At Suncrest, our services are aimed at:
